Goal: The primary goals of KNOW THE LAW! include: to improve young people's awareness of legal issues, including both their rights and responsibilities; to help them make positive decisions and resist negative peer pressure; to teach participants to use their bodies, voices, and imagination as actors; to help participants learn basic theatre vocabulary and stage directions; to improve participants' self-confidence, promote high self-esteem, and develop good work habits; and to encourage participants to look at themselves as positive role models for their peers, developing both leadership and collaboration skills.

Goal: The Latino Commission on AIDS focuses its efforts on HIV/AIDS in the Latino community through health advocacy, promoting HIV education, developing model prevention programs for high-risk communities, and building capacity in community organization.

Impact: As the only provider of Spanish language HIV treatment, the Latino Commission on AIDS has created a model of public health that uses training, outreach, prevention and culturally sensitive care for Latinos living with HIV/AIDS.

Goal: Linking the Interests of Families and Teachers (LIFT) is a research intervention program designed to prevent the development of aggressive and antisocial behavior.

Impact: Evidence suggests that LIFT can be a useful tool for promoting effective parenting in the home and decreasing aggressive behaviors with peers at school and on the playground. LIFT participants exhibited a decrease in child physical aggression toward classmates on the playground, an increase in teachers' positive impressions of child social skills with classmates, and a decrease in parents' aversive behavior during family problem-solving discussions.

Goal: Reduce unprotected insertive and receptive anal intercourse among HIV-negative black men who have sex with men (MSM) as well as to reduce the number of sex partners, increase consistent condom use during anal intercourse and increase testing for HIV and other sexually transmitted infections (STIs).

Goal: To increase daily fruits and vegetables servings by half in women served by WIC participants with the long term goal of reducing risk of cancer.

Impact: The Maryland WIC 5-A-Day Program shows that while multi-faceted community based interventions can effectively promote and sustain dietary change among low-income populations in order to reduce the risk of cancer, many obstacles remain in implementing such programs.

Goal: The mission of MOMS Orange County is to help mothers and their families have healthy babies by providing health coordination, education, and access to community services. MOMS Orange County’s vision is that all babies born in Orange County are healthy at birth.

Impact: Measures such as the percent of babies born at a low birth weight, percent of babies born premature, and the percent of babies admitted to the NICU were all markedly better for program participants when compared to many comparison benchmarks.

Goal: The goal of the pilot was to assess the impact on health care use of addressing patients’ civil legal problems – the social, financial, or environmental problems that require assistance from lawyers to remedy. The lawyer was embedded in the health care team and present during case management discussions to identify specific civil legal problems and to help the team better understand how to address them. Additionally, this partnership provided civil legal aid services to patients when needed in a community health care system.

Impact: This pilot shows a medical-legal partnership for the super-utilizers of healthcare can lead to efficiencies within the health care system, reduce costs, and improve health outcomes among the most vulnerable patients.

Goal: Memphis Fast Forward is the shared vision and unprecedented action plan to create good jobs, a better-educated workforce, a safer community, a healthier citizenry and efficient government in Memphis and Shelby County.

Impact: Major violent and property crime were reduced by 22.8% and 25.5%. The pool of high-potential teacher candidates increased from 21 to 1,800 in one year. More than 15,000 jobs were created. New capital investment of $4.2 billion was generated. Local governments saved more than $75 million.
